At the swimming pool, he finds an excuse not to take his shirt off. To everyone around him, he looks perfectly happy. Inside, he is quietly fighting a battle no one around him even knows about.
This is not one boy's story. It is the story of millions of men across India, and one of the most common conditions I see in my practice as a plastic surgeon in Hyderabad. The condition is called gynecomastia: enlargement of breast tissue in males. There is absolutely no reason to suffer in silence.
What is gynecomastia?
Every male is born with a small amount of breast gland tissue behind the nipple, just as females are. In girls, this gland grows significantly during puberty. In boys, it is not supposed to. When it does grow due to hormonal imbalance, particularly excess oestrogen relative to testosterone, the condition is called gynecomastia.
This most commonly develops during puberty, roughly between the ages of 9 and 19. It is far more common than most people realise. Roughly one in three males is affected to some degree. If you have it, you are not unusual. You are in a very large, very silent group.
Is it just fat, or is there actually gland?
This is the question I am asked most often in my consultation room, and it deserves a direct answer.
- If you have gained weight and your chest has grown along with the rest of your body, it may be mostly fat, sometimes called pseudo-gynecomastia.
- If you are thin, your overall weight has not changed dramatically, and you can feel something firm and hard directly behind the nipple, that is the breast gland.
In practice, most patients have a combination of both gland and fat. Pure fat-only cases are less common. The distinction matters because the gland cannot be reduced through exercise or diet. It requires surgery.
Why men do not talk about it
The physical condition is one thing. The psychological weight that comes with it is another matter entirely.
Many patients tell me that one comment, one joke at a school function, or one laugh at a swimming class changed the way they carried themselves for years. Boys who were once cheerful and confident become withdrawn. They default to dark colours, loose layers and a permanently guarded posture.
At school age, telling parents is the first hurdle. Shame, fear and embarrassment get in the way. When some boys do find the courage to speak up, they may be told, "It is normal, you are still growing" or "You are a man, do not be so sensitive." The conversation ends there.
In adulthood, I have seen patients who hide it from their own spouses for years, sleep with a vest on, undress only in the dark, and plan daily routines around hiding the chest. These are not rare stories. They are common ones.
At the gym, many men spend months doing heavy chest workouts, hoping to train the gland away. Exercise will build the muscle underneath, but it will never remove the gland. A good, honest gym trainer will tell you the same. The gland is structural tissue. It responds to surgery, not incline bench press.
When should you see a plastic surgeon?
If you have confirmed breast gland tissue, especially a firm lump behind the nipple, and it is not resolving on its own after puberty, you should see a plastic surgeon for consultation.
Medications are rarely effective for established gynecomastia. The definitive treatment is surgery, usually combining liposuction and gland excision, performed by a qualified plastic surgeon.

What happens at a gynecomastia consultation?
During consultation, I assess:
- How much gland tissue is present and how firm it is.
- The fat composition around the chest.
- Skin quality and elasticity, especially after major weight loss.
- The grade of gynecomastia, which determines the surgical approach.
- Whether skin excision or additional contouring is needed.

The surgery: what actually happens
Gynecomastia surgery is usually performed under general anaesthesia, so you are completely asleep. The procedure typically takes one to one and a half hours, depending on grade and complexity.
The standard approach combines two techniques:
- Liposuction: removes excess fat around the chest and improves contour.
- Gland excision: removes the firm breast gland tissue directly, usually through a small incision placed at the edge of the areola where it heals with minimal visibility.
I show patients photographs during consultation, including where incisions are made and what healed results can look like. For patients with significant excess skin, skin excision may also be needed. Recovery: a realistic week-by-week guide
Recovery from gynecomastia surgery is straightforward for most patients.
Day of surgery: You will have a compression bandage placed over your chest. After observation, you usually go home the same day.
Days 1 to 3: Rest at home. No strict bed rest is required. You can walk around normally. If you have work-from-home duties, you may return to light work within a day or two.
Day 4 follow-up: The bandage is removed. You can take a complete shower from this day. You will be advised to wear a chest compression garment for about three weeks. It sits under your shirt and is not visible to others.
Week 2 onwards: Lymphatic massage may begin when advised. This is an important part of post-surgery care and helps swelling, firmness and final contour.
Week 4 to 5: You can usually resume chest exercises. Building the pectoral muscles underneath can further improve chest shape.
For the first one to two weeks, avoid heavy weight lifting, chest exercises and self-driving a two-wheeler. Car driving is usually easier because it engages the chest less.
Results after gynecomastia surgery
Gynecomastia surgery is one of the most consistently successful procedures in plastic surgery because the main concern, a chest that does not look flat, is directly addressed. A flatter, more masculine chest contour is achieved in the vast majority of suitable patients.
But what I notice most in post-operative photographs is not just the chest. It is the posture. The way patients stand changes. Shoulders move back. The head comes up. The body language changes from hiding to confidence.
Gynecomastia is not your fault
Gynecomastia is not caused by weakness of character. It is not a lifestyle failure. It is not something you brought upon yourself. It is a physiological condition driven by hormonal changes that happened in your body during a phase of development you did not control.

In some adolescent boys, mild gynecomastia resolves naturally within 6 to 24 months as hormone levels stabilise after puberty. If firm gland tissue remains persistent beyond the late teenage years, it is unlikely to resolve without surgery.
