What is a sebaceous cyst?
Our skin contains sebaceous glands, which produce oil. This oil normally travels through a small duct and reaches the skin surface, where it helps nourish and lubricate the skin.
A sebaceous cyst forms when this duct becomes blocked. The gland may continue producing material, but because the outlet is blocked, the contents collect inside and the swelling gradually increases.
This is why many people notice a slow-growing lump under the skin, especially on the scalp or back.
Why does white paste-like material come out?
Some sebaceous cysts burst or discharge a thick, whitish, toothpaste-like material. This material comes from the trapped contents inside the cyst.
Even if the cyst drains temporarily, the cyst wall or sac may remain inside. When the opening blocks again, the cyst can refill and come back.
Signs that a sebaceous cyst is infected
A sebaceous cyst may become infected, especially after it bursts or is repeatedly squeezed. Common signs include:
- Increasing pain.
- Redness around the lump.
- Warmth or swelling.
- Pus discharge.
- Tenderness when touched.
An infected cyst needs medical assessment. Sometimes antibiotics and drainage are needed first, followed by definitive removal later.
Why surgical removal is usually the permanent treatment
Once a sebaceous cyst has formed, the affected gland and cyst sac usually do not return to normal function. If only the contents are squeezed out, the sac can remain and refill.
Permanent treatment generally requires surgical removal of the entire cyst along with its sac. This is a small procedure in most cases.
How is sebaceous cyst removal done?
Sebaceous cyst removal is commonly performed under local anaesthesia. This means the area is numbed with an injection, and the patient stays awake and comfortable.
The surgeon makes a small incision over the cyst, removes the cyst and its sac completely, cleans the area, and closes the wound. Antibiotics may be prescribed when there is infection or risk of infection.

Should you squeeze a sebaceous cyst at home?
It is better not to squeeze or puncture a cyst at home. Squeezing can rupture the cyst wall, push contents deeper into tissue, increase inflammation, and raise the risk of infection.
If a lump is growing, painful, red, repeatedly discharging, or cosmetically bothersome, a consultation is safer than repeated home drainage.

How do we know it is infected? There may be severe pain, redness in that area, and pus discharge.
