Many men have puffy nipples despite regular gym training and a slim build. In these cases, a firm gynecomastia gland behind the nipple may push the areola forward. The cause can be chest fat, glandular gynecomastia, or a combination of both.

Fat versus gland

Chest fat is usually soft and diffuse. True gynecomastia includes a firmer gland beneath the nipple and areola. Exercise can reduce fat, but established gland tissue may remain and continue to make the nipple look puffy.

How treatment is decided

A clinical examination determines whether the chest contains excess fat, gland, or both. Many men have a combination. When appropriate, treatment may combine liposuction with gland excision to reduce nipple puffiness and contour the chest in a more masculine shape.
